To place orders:

1. Click link below
2. Print order form (if ordering more than one line or delivery please print desired amount).
3. Make sure all orders are written with store name, ship to, bill to, style number, size scale and delivery.
4. Create PO#
5. Fax all orders to 214-630-4215.
